I was asked to perform a colonoscopy on a young female day care worker. She had been complaining of abdominal discomfort, nausea and irregular bowel habit for months. Her physical examination was essentially unremarkable. Our clinical impression was that she probably had irritable bowel syndrome but she had not responded to usual measures.
The colonoscopy prep was poor and made the examination difficult. When the scope reached cecum, we found worms floating around. This is pinworm or Enterobius vermicularis. The diagnosis was confirmed by microscopy.
Besides frequently being asymptomatic, the most common manifestation is peri-anal itching. However, it has potential to cause a wide range of gastrointestinal and non-gastrointestinal manifestations, including potentially life-threatening illnesses.
